A “Power of Attorney” is a document that authorizes another to act on your behalf as your agent or attorney on matters which are contained within the document.
A Health Care Power of Attorney is meant to allow a person to act on your behalf concerning medical matters. For medical needs short of end of life issues, if an individual is non-communicative, a Health Care Power of Attorney action allows you to name someone to make health care decisions for you. The person named is able to receive information and direct which medical providers are used for your care and to authorize any procedures that may be necessary.
